HOW WE COLLECT
Payment For Services
We accept payment through our credit card vendor with Visa, HAS cards, checks or cash. You can also pay using Zelle.
Due to the extensive complications of billing, we cannot bill private insurance, Medicaid, or Medicare. Upon request, we can prepare a superbill — a detailed form containing the services provided — which you are free to submit to your insurance carrier for reimbursement.
OUR STANDARD RATES FOR SERVICES:
Advanced Care Planning, Palliative Medicine, Wound Care, and Home Visits
$375 per hour
Telemedicine Appointment
$200 per hour
End of Life Consultation
$500 for two hours
Ketamine Treatment
$600 per session
Medical Aid in Dying
If your income is within 20% of the federal poverty line, please let us know.
